What is Holistic Care?
Holistic care is a term that deserves a pause to consider the deeper meaning. When you work with your care manager, holistic care is a core value you can expect. To get the best care, all the needs of the person must be reviewed. It is not enough to just look at the medical situation while ignoring financial or mental health issues. A professional care manager knows to look at the entire person.

The social work case manager engages the client (and, when appropriate, other members of the family system) in all aspects of case management and tailors the services to the client’s needs, preferences, and goals.
The social work case manager understands that each individual experiences a mutually influential relationship with her or his physical and social environment and cannot be understood outside of that context.
The social work case manager does not work in isolation. Collaboration with other social workers, other disciplines, and other organizations is integral to the case management process.
