Hi! I’m Jessica L. Raitz, MSW

PROFESSIONAL GUARDIAN, FOUNDER & PRESIDENT OF JESSICA CARE

With over 15 years of experience in the healthcare industry, I focused within the geriatric population.

I have in-depth experience assisting individuals, and their loved ones, to work through the complexities associated with assisted living facilities, cognitive impairments, and unexpected hospitalizations.

I am proud to be an Aging LifeCare Professional and I enjoy my role as a geriatric care manager.

I’m here to help you and your family too.

This is my story…

I have been fortunate to work closely with the best medical professionals at the area hospitals including Manatee Memorial, Lakewood Ranch Medical, and Complex Care.

These well-vetted mentors are now my strategic partners, assisting my clientele to find solution-driven care plans.

Having “cut my teeth” as a discharge planner in these hospitals, many times dealing with a crisis, has made me well-equipped to help my clients navigate through their healthcare issues. I enjoy being able to discharge patients from the hospital to their home, with a successful outcome.

Although I am in the healthcare industry, I am a social worker at heart. Patient advocacy is at the core of what I do.

To better protect my clients, I have earned my Professional Guardian designation. My background as a social worker allows me to leverage additional resources for my clients, while also assisting adult children that are often dealing with complex emotions that come with an elderly loved one.

I understand how to navigate the complicated nuances of health insurance plans making sure my clients get every possible benefit those plans may offer.

Since leaving the hospital system, I have applied my knowledge, and I love using this knowledge to help my clients.

Jessica served as a case manager at Manatee Memorial Hospital and Lakewood Ranch Medical in Bradenton, Florida.
Jessica holds a master’s degree in Social Work from The Catholic University of America and B.A. from Florida State University.
She was recognized by The International Honor Society of Social Sciences.
